Graduation Packet Paperworkrevised 4/7/08
The following forms are all part of the Graduation Packet. Please see Graduation Procedure for a step-by-step description of the graduation process, which must be completed for each graduating student, at the beginning of his/her last semester.
According to the date assigned on the ES Meetings & Paperwork Deadlines calendar, the following items should be submitted as the Graduation Packet to the Educational Support Manager, for each graduating student:
-Student Graduation Approval Request -Graduation Requirements Worksheet -Transcript (color-coded, must include all semesters, except the last, on a GVCS transcript) -Portfolio (for January Graduates only)
Student Graduation Approval Request: Please fill out all information, including parent and student signatures.
Graduation Requirements Worksheet This form is used for tracking the student’s completion of the graduation requirements within each growth area. The ES begins this form when a high school student is first assigned to them and keeps this form updated, and in the student's folder, as the student completes their courses each semester. It is helpful for the ES to complete this form on a regular basis (each semester) to make sure that the student is "on track" and will be able graduate within the agreed upon time frame. It can be quickly compared to the transcript to be sure all courses taken are showing on the transcript. Transcript information from transferring high school students is added onto the Graduation Requirements worksheet by the ES. The information on the transcript from the prior school is important in planning classes, if the student is interested in earning a high school diploma with GVCS. As soon as the official transcript for this student is in Teaams, compare the information you were given by the student/parent to those courses on the official transcript. Keep an updated copy of the Graduation Requirements Worksheet in the file you keep for each high school student. This student file could also include other pertinent graduation documentation you have acquired while serving the student: current transcripts, copies of any graduation paperwork submitted to the office, etc. Keeping your information current will make any transfer to another ES a smooth process.
Complete/Final Transcript: You will submit a High School Course Selections form & appropriate course approval paperwork for your potential graduates, the same as you do for each of your high school students. Once those courses are approved, those course titles will be added to your potential graduate’s semester course list in Teaams. You will be able to print a transcript for the grad packet, however that transcript will not have the final semester’s courses, grades and credits for the student. Those grades and credits will be entered directly into Teaams by the ES, upon the successful completion of the final semester’s work. The final transcript must have any previous school’s transcripts entered by GVCS, and must include all coursework entered on the Graduation Requirements Worksheet. The ES will email the ES Advisor & Educational Support Manager at the end of the semester, to notify them of the student’s successful completion of the graduation requirements and that the ES has submitted the hard copy attendance rollsheet and final attendance and LR in Teaams, as well as entered the grades & credits. At that point, if all of the graduation paperwork is complete and accurate, the Educational Support Manager will print the final and complete transcript and give final approval for graduation. The ES must also inform the Educational Support Manager if the student does not successfully complete all the necessary requirements by the end of the semester. In this case, the student will remain enrolled in the school and continue attempting to meet those final requirements.
Disenroll Notice: When the student completes all of his/her graduation requirements, the ES sends the “Disenroll – Pending” email to the appropriate people. Make certain to include special ed on the pending disenroll email, if the student is special education. The ES informs the Ed. Support Mgr. and the ES Advisor once s/he completes all the records for the graduate, i.e. report card, learning record, and attendance record. The Ed. Support Mgr. will import the report card, double check everything and send the final disenroll email. If the Graduation Packet is incomplete, the Educational Support Manager will return the whole packet to the ES, and the student will not be disenrolled until the packet is complete and approved.
